Please note that the below facts to make a claim.

  • All necessary information must be filled.
  • The claimant must sign in the digital claim form to make a claim.
  • Claim officer from AMI Life will contact for claim process.

Important Notes

  • The digital claims form should be filled out completely for the claims process. Please be aware that any dishonesty and misrepresentation may render refusal of your claims.
  • All relevant documents pertaining to your claims are properly attached in the designated upload section.
  • It is necessary for your signature to be documented on the e-claims form.
  • For Endowment Life, the original policy contract should be sent to our AMI Life Office to proceed the benefit payment process.
  • If any of the requirements mentioned above are not fulfilled, we will reach out to you and kindly ask for additional information. We appreciate your cooperation in this matter.
